Before completing the form below make sure you have:
- Liability insurance
- Automobile insurance
- Worker's compensation insurance
- Job references.
Once approved we will ask you to send us the following document:
- W-9 Form. Copy of the W-9 form could be found here.
- Certificate of Insurance from your insurance agent. The Certificate Holder should be; "Interworld Cleaning Inc, 24 Roberts Ave, Catonsville, MD 21228"
- Signed Subcontractor Agreement. Will be provided from Interworld Cleaning
- Sample price-list, if you have one
